This questionnaire can be filled in by the manager in a situation where they can clearly tell what is being said by client and sales person. It can also be used in role play situations.

The questions are prompts and all areas should be discussed as appropriate. Make notes and have specific examples to make the discussion fairer. If you want to use it on yourself it’s not as effective as having someone to discuss issues with but will still be useful if you can be as objective as you can.
